Show DES TUMEURS IIYPEB'l‘ROPHIQUES DES MEMBRANES MEQIÎEËSES. 265 ment est libre, ne reste pas toujours simple, et elle montre une certaine disposition à l'aspect lobulé ou rameux, ce que nous avons surtout observé dans certains petits polypes dermo-muqueux du col utérin, ainsi que dans les polypes glandulaires de l'intestin grêle. La surface de ces tumeurs varie selon leur origine; elle est lisse dans l'hypertrophie muco-dermique et sous-muqueuse, tandis qu'elle est grenue et irrégulière, comme framboisée, quelquefois verruqueuse dans l'hypertrophie épithéliale. Elle est poin- tillée, présentantà sa surface une multitude de creux, souvent déjà visibles à l'œil nu, dans l'hypertrophie muco-glandulaire du rectum, tandis que dans les polypes où les glandules sont closes, on voit plutôt une multitude de grains saillants, mais à côté d'autres pourvus d'orifices bien distincts. La couleur de ces tumeurs est, en thèse générale, celle des membranes muqueuses, pâle et jaunâtre si Ell- elles sont peu vasculaires, veinée de rouge ou d'un rouge plus intense si la vascularité est considérable, d'un gris de fer ou noirâtre lorsqu'il y a mélange avec des dépôts pigmentaires, comme c'est le cas dans ici laire à aréoles distinctes. Cela s'observe surtout dans les polypes muco-glandulaires du col de la matrice. Il est important de noter que les tissus qui entourent ces productions se trouvent souvent dans un état d‘hypérémie habituelle; leurs vaisseaux peuvent se dilater et donner lieu ainsi à des hémorrhagies fré- Il] ' quentes. Leur grand volume peut gêner les parties voisines, soit en les atrophiant par compression, soit et en les disjoignant, comme cela arrive quelquefois pour les parties molles et osseuses des cavités nasales. Nulle part leur danger n'est aussi grand que dans le pharynx ou dans le larynx, parce qu'elles peuvent y intercepter le passage des aliments ou celui de l'air, et dans ce dernier cas surtout, entraîner la mort par suffocation. ' Les polypes peuvent s'accroître sans se modifier beaucoup dans leur intérieur; mais une fois que leur volumeaatteint une certaine dimension, on voit leur surface quelquefois s'enflammer, s'ulcérer, subir .ä _ AA;A»AA,_ v.;‘r ".jTà l'?.'!‘.‘."l' _. les polypes de l'intestin grêle. Dans quelques endroits on trouve leur intérieur d'une structure trabécu- zur une espèce de fonte putrilagineuse et même gangréneuse. Ces masses peuvent se détacher spontanément pes en partie ou en totalité, et sont alors rendues avec les excrétions. tm- En général, les polypes restent pendant longtemps à l'état latent et ne commencent à devenir une maladie vraiment sérieuse que lorsqu'ils ont produit ce développement vasculo-érectile de leur voisinage, qui entretient des hémorrhagies, ou lorsque, par leur siége, ils mettent obstacle à l'accomplissement de quelque fonction indispensable à l'entretien de la vie. Quant à la cause qui produit les polypes, nous avons eu autrefois quelque tendance à l'attribuer en grande partie à une inflammation chronique; aujourd'hui nous ne regardons cet élément étiologique que comme l'exception, et nous sommes de plus en plus disposé à n'y voir qu'une nutrition locale exagérée dont la dernière cause nous échappe dans la majorité des cas. Nous allons suivre'les polypes dans les divers organes qui leur donnent naissance, savoir : dans les fosses : les 1° Les polypes des fosses nasales sont ordinairement multiples; ils prennent leur point de départ à leur paroi supérieure et externe, cependant on les voit quelquefois provenir du cornet inférieur. Dans 20 cas j'ai noté l'âge et le sexe des malades; sur ce nombre il y avait 12 hommes et 8 femmes, ce qui n'établit pas une grande différence. Quant à l'âge où les malades ont réclamé l'opération, n au rmeS C'était deux fois entre 10 et 20, onze fois entre 20 et 30, quatre fois entre 30 et 40, une fois cntreàO et d, et 50, et deux fois entre 50 et 60, ce qui établit une prédisposition marquée pour la seconde partie de la lance PP"' jeunesse, entre 20 et 30 ans surtout.
