Lorsque la maladie est avancée, la face de ces malades présente souvent l'aspect cyanotique qui accompagne la dyspnée; mais elle est quelquefois mêlée d'une apparence cachectique prononcée. L'examen du cœur montre, dans la majorité des cas, une augmentation de matité en rapportavec le degré d'hypertrophie ou d'épanchement péricardique. L'emphysème peut complétement masquer la matité ainsi que les autres signes de maladie du cœur. L'impulsion est souvent plus forte et les accès de pa1pitaliuu5 peu peu ces existent plus fréquemment dans la première partie de la maladie. Une voussure notable à la région précordiale a été constatée quatre fois; un frémissement vibratoire prononcé vers la pointe a existé cinq 000 25 vait fois. Les douleurs étaient rares, dues ordinairement a une névralgie intercostale. Les bruits anormaux du cœur ont tout particulièrement fixé notre attention comme un sujet encore en litige. Dix-sept fois nous avons constaté un bruit de souffle au premier temps, et quatre fois aux deux temps; nous n'avons pas constaté son existence au deuxième temps seul, quoique ce fait ait été signalé par de bons observateurs. Chez trois de nos malades il n'y avait point de bruits anormaux, quoique le mal eûtété bien prononcé; chez ces malades, les battements du cœur étaient très irréguliers. Le maximum du bruit anormal était ordinairement a la pointe et à gauche ; cependant, le plus souvent, ce bruit se prolongeait vers la base, et même jusqu'à l'origine de l'aorte. l.c soullle était tantôt doux, tantôt plus 011 moins rude, râpeux, et plusieurs fois musical, même avec piaulement.
