Bouillaud signale comme appartenant à l'endocardite aiguë, c'est la faculté de produire une coagulation du sang dans le cœur de la même manière que l'artérite et la phlébite. C'est là un des paradoxes que M. Cruveilhier a introduits dans la science, et que M. Bouillaud paraît avoir adopté; mais rien de plus contestable que l'origine inflammatoire des concrétions fibrineuses plus ou moins enchevêtrées entre les colonnes cbarnues et les tendons valvulaires ou accolées aux parois internes du cœur. On les rencontre dans des maladies trop diverses pour qu'on puisse y voir le moindre caractère spécial, et nous n'avonsjamais vu des vaisseaux y pénétrer, comme M. Bouillaud prétend l'avoir observé. . Dans toute espèce de maladies, les concrétions librineuses sont plus ab0ndantes dans les cavités droites du cœur que dans les cavités gauches. Toutefois, à cette occasion, nous prenons acte de l'aveu de l'auteur, qui (lit page 309: « il me paraît prouvé par des faits nombreux que ces petites masses » librineuses peuvent s'organiser et se transformer en végétalions ou en granulations. » Nous démontrerons ailleurs que la phlébite et l'artérite n'exercent pas davantage une action coagu- lante constante et prompte sur le sang, et que, dans ces cas, on a commis une double méprise: 1° celle de croire toujours atteints d'une phlébite ou d'une artérite les vaisseaux de ce genre qui renfermaient des caillots adhérents; "2" cette autre, plus inconcevable encore, de ne tenir aucun compte des nombreux &… cas de phlébite dans lesquels on trouvait même du pas sur la paroi interne des veines, sans qu'il y eût trace de coagulation sanguine. Nous reviendrons sur ce point a l'occasion des maladies des vaisseaux. La seconde période de l'endocardile est, d'après M. Bouillaud, celle de l'épaississement hypertrophique des tissus et de l'organisation des produits sécrétés ou d'une portion des concrétions fibrineuses, avec ou sans adhérence anormale a l'intérieur du Cœur. Lorsque l'endocardite se prolonge pendant quinze, ter eff< val CO] vingt et trentejours etau delà, les tissus enflammés s'épaississent, et il se forme ainsi des boursoutlements fongueux ou des végétations granuleuses, surtout sur le bord libre des valvules, ou des adhérences cellulofibreuses. Quoique les végétations ou granulations se rencontrent aussi quelquefois à la face interne des oreillettes, elles ont cependant une prédilection pour les valvules et leur bord libre, et affectent une forme globuleuse ou verruqueuse. M. Bouillaud voudrait désigner les premières sous le nom d'albumineuses ou fibrineuses, et les secondes sous celui de cornées ou de cartilagineuses; mais cette distinction ne repose ni sur des observations histologiques ni sur des analyses chimiques, qui seules pourraient autoriser une pareille distinction. Pour nous, qui n'admettons l'origine inflammatoire de ces végétations qu'avec réserve, nous assimilons plutôt les végétations globuleuses aux coagulali0ns fibrineuses, et les Végétations dures, plus tard ostéoïdes, a une altération analogue a l'athérome des artères; toutefois, les unes ou les autres peuvent avoir pour point de départ l'inflammation. Les végétations albumineuses ou fibrineuscs sont molles, se détachent facilement, et oiirent une colo- car ration qui varie entre le blanc jaunàlre, le rosé et le rouge. Les végétations verruqueuses sont très rét adherentes et offrent quelque ressemblance avec les végétations des organes génitaux; elles font corps: pout ams1 dire, avec les parties sous-jacentes, leur tissu est comme corné, de la structure du fibro- , .. . suŒÿJ-:‘ÊÂ. ‘ + .; .,. , |
