Parmi les altérations rétro- grades, une qui n'est pas rare est la flétrissure de quelq ues lobules apoplectiques; leur substance, alors, devient plus sèche, se remplit de sels c alcatres qui restent comme dernier résidu du sang épanché, et ces concret10ns, entourees d un pigment plus ou m01ns foncé, ont pu plus1eursf01s en imposer pour des tubercules guéris. On observe l'apoplexie pulmonaire moins rarement dans la phthisie qu'on ne devrait s'y attendre, malgré la fréquence de la pneumorrlragic dans cette affection. La majorit é des cas, par contre, dans lesquels on la trouve, appartient aux maladies organiques du cœur, et princi palement aux affections de la valvulc mitralc. Aussi en a-t-il déjà été question à cette occasion. La gangrène des produits accidentels, le cancer mon, les parasites 'hydatiqu es, provoquent également, dans un certain nombre de cas, la rupture d'un vaisseau qui donne lieu à l'hémopty sie et à l'apoplexie de cet organe. Toutes les maladies dans lesquelles le sang est profondément altéré peuvent la produire. C'est ainsi qu'en 1847 nous avons vu plusieurs fois le scorbut donner lieu a la production de foyers sanguins dans les poumons, pendant l'épidémie qui a régné a cette époque à la Salpêtrière. Pendant l'été de 1853, nous avons observé à Zurich plusieurs cas de fièvre typlioïdé grave, avec nombreuses pétéchies, et dans deux autopsies, nous avons rencontré une infiltration sanguine de toutun lobe pulmonaire. Nous avons publié (1) un travail sur les cas graves et ordinairement mortels d'ictère, que nous désignons sous le nom d'ictêre typhot'de. Il existe dans cette affection une tendance prononcée à l'hérnorrhagie, et dans plusieurs des observations, il y avait des foyers apoplectiques dans les poumons. Dans l'anévrysme de l'aorte, on observe le sansr réuni en masse sous deux formes et dans des circonstances tout à fait dif- férentes. Quelquefois l'anévrysme s'enkyste par suite de cette disposition curieuse, en vertu de laquelle l'anévrysme se creuse une cavité dans une partie du poumon; après la disparition des tuniques arté- rielles, le poumon forme alors une véritable coque aux caillots membraneux du sang. Ce n'est pas la une véritable apoplexie. flous avons été frappé de la fréquence de l'apoplexie pulmo- naire dans les expériences sur la pyohémie, dans lesquelles nous avons souventrencontré des foyers hémorrhagiques après l'injection du pus dans le torrent circulatoire; et même les abcès pyohémiques, chez l'homme. montrentà leur origine une infiltration sanguine autour des vaisseaux hy‘pérémiés et autour de l'infiltration purulente. On voit par ces remarques à quel point les causes des épanchements sanguins pulmonaires peuvent varier, et combien il faut par conséquent se garder d'adopter des opinions trop exclusives sur les causes de l'hémoptysie, que l'on est porté généralement à attribuer, avec M. Louis, aux tubercules pulmonaires.
