On observe bien souvent, il ne est vrai, dansles scrofules, des dermites pustulo-crustacées du cuir chevelu; mais la vraie teigne est. une tout autre maladie, et consiste dans le développement d'un parasite du règne végétal. de- Les maladies de la peau chez les scrofuleux se rencontrent bien plus souvent sur les sujets qui n'offrent our 1tré point de complication tuberculettse et sur ceux qui ont en même temps une tuberculisation glandulaire ou autre; elle se complique souvent d'autres formes de scrofules, telles que des ophthalmies, des maladies du système osseux, des affections articulaires, des ulcères et des abcès, etc. Les proportions de ces com- 1tre plications ont été indiquées avec des chiffres exacts dans le chapitre spécial consacré aux dermatoscs er- chez les scrofuleux, affections qui constituent, du reste, la localisation la plus bénigne des scrofules. ' 2" Le tissu cellulaire sous-cutané n'est que d'une manière tout à fait rare et exceptionnelle le siège d'un dépôt tuberculeux, tandis que chez les scrofuleux, on observe souvent des abcès, des ulcères ou des listules dans les lames profondes du derme ou dans le tissu cellulaire sous-dermique. Ces affections sont au )lus pas z le souvent multiples ou se montrent successivement sur diverses régions du corps. Nous avons de plus ; et remarqué que ces abcès, avec leurs divers modes de terminaison, alternent sement avec d'autres formes de scrofules, et plus rarement avec une tuberculisation glandulaire externe. Parmi bien d'autres caractères, il y en a un qui mérite toute l'attention des médecins: c'est que les ulcères tuberculeux des glandes ont leur siège de prédilection au cou, plus rarement aux aines et aux aisselles, tandis que les abcès froids siégent de préférence au tronc et aux membres. Le pas de ces derniers est séreux et contient des concrétions comme caillebottées, tandis que dans celui des abcès tuberculeux, qui est beaucoup plus épais, on reconnaît ordinairement des morceaux de matière tuber-r culeuse non encore dissoute. 3" Les organes des sens. Les yeux, dans une plus grande proportion que les autres organes, sont une des parties fréquemment atteintes chez les scrofuleux, tandis que les tubercules ne peuvent guère les atteindre. L'ophthalmie seule s'est rencontrée chez près d'un cinquième des scrofuleux soumis a notre observation. Les diverses lésions qu'on observe dans cette maladie, chez les scrofulcäux, ne présentent point une spécificité distincte; l'ensemble des phénomènes qu'elle offre soit dans l'état local, soit dans l'état général de ciaux coïncider ou alterner. L'otorrhée, fréquente chez les scrofuleux, est surtout grave lorsqu'elle dépend d'une elite interne; men- elle provoque alors une surdité complète, au moins du côté affecté, un écoulement fétide mêlé de par- ana- celles osseuses; dans quelques cas, une paralysie faciale, dans d'autres des abcès mastoïdiens; elle peut ;naler même devenir mortelle en donnant lieu à des accidents cérébraux, à une phlébite des sinus, à l'infec- rence ez les tion purulente ou a des abcès du cerveau. lt" Les membranes muqueuses ne subissent guère d'altérations profondes produites par les scrofules. On yobserve bien une viciation de sécrétion, de l'irrégularité dans les fonctions digestives par exemple, 36 Q… d'autres fois une inflammation chronique avec sécrétion n…co-purulente; mais encore celle-ci se issent ; pus- constate-telle de préférence sur les muqueuses les plus rapprochées de la surface du corps, telles que la membrane pituitaire ou la membrane muqueuse vaginale. La conjonctivite, souvent partielle, s'accompagne d'une ou de plusieurs phlyctènes au bord cornéal de la conjonctivc; la kératite ulcéreuse est plus rare, la photophobie est un des symptômes fréquents. La marche de cette ophthalmie est irrégulière et très chronique, elle est un peu plus fréquente chez les petites filles que chez les jeunes garçons. C'est de toutes les formes de scrofules celle qui complique le plus volontiers les tubercules glandulaires externes (dans un quart des cas).
