Les indurations partielles uniformes que l'o n rencontre dans le lipome et que l'on constate à travers les téguments avant l'opération ne sont, la plupart du temps, qu'un fort développement du tissu fibro-cellulaire; mais il y a un autre ordre d'induration qui a frappé les anatomo-pathologistes, sans avoir été expliqué d'une manière satisfaisante jusqu'à ce jour. (Le sont ces concrétions dures, ossiformes, dont l'existence a été signalée un certain nombre de fois au milieu de lipomes. Jamais on n'y trouve de véritable substance osseuse ni cartilagineusc; ce sont des concrétions calcaires avec quelques débris fibri- neux, qui, d'après toutes nos études sur ce sujet, ne peuvent ètre autre chose que des résidus hémorrhagiques. L'inflammation du lipome a été également observée un certain nombre de fois. Abernethy parle d'un abcès qu'il a trouvé au milieu d'un lipome. Les cas de ce genre se sont multipliés dans ces dernières années. M. Michou (l) a présenté en 1846, à la Société de chirurgie, un lipome très volumineux qu'il a enlevé à la cuisse d'une femme de trente ans, forte et bien portante. Cette tumeur, qui avait présenté de la fluctuation, fut d'abord ponctionnée; il en sortit environ un verre de pas liquide, à la surface duquel il y avait de la graisse fondue; la tumeur fut alors enlevée en totalité, et sa nature lipomateuse mise hors de contestation. M. Hébert (2) cite un fait observé dans le service de M. Morel-Lavallée, à l'hôpital de la Charité, ayant trait à une femme âgée de cinquante-deux ans, qui portait depuis longtemps un lipome dans le creux de l'aisselle gauche. Dans les huit jours qui précédèrent son entrée a l'hôpital, elle vit se développer une tuméfaction circonscrite et saillante, accompagnée (l'élancements douloureux; la peau n'avait point rougi, la fluctuation était obscure. Une incision pratiquée donna issue a du pus contenu au centre d'un lipome, qui, lui-même, a été presque entièrement détruit plus tard par l'inflammation suppurative qui s'en était emparée. ext J'ai décrit (3) un cas curieux d'ulcération d'un lipome: c'était celui d'une femme de cinquante-six ne ans qui portait depuis vingt ans, a la fesse droite, une tumeur de ce genre qui avait acquis un dévelop- l'il pement considérable depuis la cessation des règles. Six mois plus tard, l'ulcère se cicatrisa, et l'œdème des membres reparut, l'extirpation fut faite, et la malade guérit complétement. La gangrène des lipomes est di plus rare encore que son inflammation. se: M. Hébert cite, a cet égard, l'opinion de M. Velpeau (Æ), suivant lequel le lipome ne subit pas de 30 transformation, mais peut se gangréner et tomber en putrilage; il rapporte ensuite un cas qui lui a été en communiqué par M. le docteur Bertherand.
