Dans l'abdomen, elles forment un paquet depuis l'angle sacro-vertébral. Chaque glande a de 3 a l centimètres de longueur- AU 0011, dans l'aime et dans dc tr es de longueur sur Vaisselle droite, il y en a de très engorgées, dont l'une atteint jusqu'à 8 centimètr 110 5 d'épaisseur. Dans deux des glandes abdominales, on trouve un corps enkysté de nature fibrineuse. re! Dans le second fait, un homme succombe a la suite d'une diarrhée chronique et d'anasarque. Outre hypertrophie un léger ramollissement de la muqueuse du côlon, on ne trouve d'autre altération qu'une BH for ation considérable d'un grand nombre de glandes mésentériques. Quelques-unes ont subi la transform tro dl libreuse, mais aucune ne renferme ni tubercule ni cancer. exc On voit, par ces faits, que l'liypertrophie des glandes lymphatiques internes est une affection grave. Il n'en est plus de même lorsqu'elle porte sur les glandes superficielles; mais ici encore elles offrent une tendance a la multiplicité, ce qui indique une disposition diathésique. Nous avons recueilli 11 cas d'hypertrophie des glandes lymphatiques superficielles : 6 fois le mal siégeait sur divers points du cou; Piel gén ] l0Pl 3 fois dans les aisselles, 1 fois dans l'aine et 1 fois au milieu du bras. POU .Il faut distinguer deux formes, dont l'une consiste dans l'exagération des éléments normaux, tandis que dans l'autre, il se forme plutôt des éléments connectifs, soit fibre-plastiques, soit fibreux. C'est par leur agglomération que ces glandes peuvent former des paquets du volume de l'un ou des deux poings. Une seule glande peut atteindre celui d'un œuf de poule. Leur forme est ordinairement ovoïde, aplatie, Le ' ressemblant à une amande, à un rein, etc. Ces tumeurs sont mobiles, la peau est intacte à leur niveau. La | ' vascularité est peu considérable ou inégale, forte par places, même avec épanchements sanguins. Sur une â:n coupe fraîche, le tissu est d'un jaune rosé, grenu ou lobulé, luisant, quelquefois gélatinif0rme, avec ou sans îlots vascularisés; on le trouve parfois d'un jaune pâle, d'un aspect fibreux. Ce tissu, en s'atro- '_ par phiant, devient terne et peut ressembler au tubercule. Les lymphatiques sont ordinairement très déve- loppés %] la surface. Lorsque le parenchyme glandulaire est principalement hypertrophié, il renferme un suc trouble qui ï:{î tient en suspension les cellules et noyaux, si constants dans ces glandes : les premières de ‘l/100€ de mil- le n limètre de largeur, plus grandes lorsqu'elles renferment cinq à six noyaux ; ceux-ci, de 1/200e à 1/120" éb de millimètre, munis d'un nucléole punctiforme. Nous avons vu quelquefois dans ces glandes hyperlro- S ] phiées une espèce d'ébauche dev vésicules glandulaires de 1/8" à 1/4 de millimètre. Une trame fibroïdey manque rarement. Les éléments libro-plastiques et lihreux peuvent occuper presque exclusivement la tumeur. Des granules graisseux se trouventsurtout dans les parties atrophiées et phymatoïdes. Nous (] l'ii do regardons cette diathèse hypertrophique comme complétement indépendante de la tuberculisation, avec enf laquelle elle peut cependant coïncider. N'oublions pas de noter qu'on trouve quelquefois des kystes à l'intérieur de ces glandes turnéfiées. Ce n'est qu'en devenant trés volumineuses que les glandes lympha- fair oise tiques hypertrophiées adhèrent aux parties voisines, compriment les muscles et les vaisseaux; mais leur di . influence sur la santé générale est ordinairement nulle. °V3 ‘ '…Ï r-VI' Cette maladie est plus fréquente chez l'homme que chez la femme. Dans notre relevé, sur 12 cas, …; y compris ceux de M. Cossy, il y avait 9 hommes et 3 femmes. L'âge a été noté huit fois. Sur 00 nombre, il y en a deux de 15 a 20 ans, deux de 20 à 25, trois de 30 à 35, et un seul de 62. C'est donc après la puberté, de 20 a 35 ans, que la maladie s'observe de préférence.
