Show 119 school of medicine and health sciences In recent years, the United States has seen a dramatic increase in the annual number of total joint replacement surgeries performed. Periprosthetic Joint Infections (PJI) pose a serious threat to surgi-cal outcomes and result in costly revision procedures. Increasing incidence of antibiotic resistance has led to several studies exploring antimicrobial peptides and synthetic peptide analogues for their bactericidal potential and decreased susceptibility to acquired bacterial resistance. These studies have demonstrated the potent bactericidal activity of these compounds. Thus, it was conceived that PJI might be reduced with the perioperative introduction of synthetic antimicrobial peptide analogues. In this study, it was hypothesized that combination coatings, prepared from medical grade silicone polymer and Cationic Steroidal Antimicrobial-13 (CSA-13), a synthetic peptide analogue, would treat and prevent bacterial contamination when applied to titanium bone implants in vitro. Furthermore, it was hypothesized that the polymer released CSA-13 antimicrobial would demonstrate greater bacteri-cidal potential than conventional antibiotics against heavy inocula (5x108 colony forming units (CFU)) of methicillin-resistant Staphylococcus aureus (MRSA). Combination coatings were prepared in a weight-to-weight ratio of 18% for all groups. Group one con-tained the CSA-13 antimicrobial; Group 2 was loaded with vancomycin, a common antibiotic used to treat MRSA infections. Group 3 used daptomycin, an antibiotic that is used to treat MRSA infections in a hospital setting. Finally, Group 4 consisted of combination coatings loaded with tetracycline, which is used to treat MRSA within the community. Preliminary results have shown that all bacteria were eliminated using standard inocula (1x105 CFU) in all groups. This research is currently underway, and tests with heavy inocula are currently taking place. These results will be available by the spring of 2012.
