Sweazey, <br> Salt Lake City, Utah. <br><br> My dear Mr. Sweazey:- <br><br> We are sending you through the mail the Annual Report blanks. Enclosed with each blank are instructions for compiling the same, which I trust you will carefully observe. <br><br> The reports are to be made out and forwarded not earlier than March 15th and not later than March 18th, 1905. I give you timely notice, so that you may be prepared, when the blanks are received, to fill out the same. <br><br> Should the blanks fail to reach you by March 1st, notify me at once, and duplicates will be sent you. <br><br> Very sincerely yours, <br><br> Geo.
