Abstract An elderly man developed acute progressive supranuclear ophthalmoplegia and other central nervous system manifestations that suggested Whipple disease. Results of small intestinal biopsy were negative but polymerase chain reaction testing of the cerebrospinal fluid confirmed the diagnosis.
