Presenting Symptom: weakness of conjugate gaze to either side; good vertical movements; considerable improvement after thiamine treatment, but some residual lag of the adducting eye. 57 year old man with ataxia, confusion, and weakness of gaze to the right; history of high alcohol intake and poor diet. Anatomy: Oculomotor Muscles. Disease/ Diagnosis: Wernicke Encephalopathy; Thiamine Deficiency
