Implementing a standardized screening tool to identify unmet needs in breast cancer survivors increased the identification of needs and utilization of survivorship resources. Background • Breast cancer survivors struggle with physical, psychosocial, and practical concerns (Hussey et al., 2024) • National Guidelines recommend screening for unmet needs in post-treatment patients (Sanft et al., 2023) • Standardized screening tools identify survivors with unmet needs (McDonough et al., 2021) Methods Results • Surveyed four providers to understand current screening practices, workflows, and utilization of survivorship resources • 94% of eligible patients were screened. 92% of screened patients reported at least one unmet need • Conducted a retrospective chart audit to determine referral rates • 42% increase in referrals to survivorship resources • A clinic workflow was created to implement a standardized screening tool • 75% of providers found the protocol feasible and were satisfied, 25% were neutral • Education was disseminated to involved medical assistants and providers • 100% of providers found the tool easy to use Purpose Number of patients • Screening helped identify needs, improved communication with patients, and improved care 144 Conclusions Patients screened 134 • The standardized screening tool identified 92% of patients with unmet needs Patients who reported at least one unmet need 123 • Screening increased referrals to survivorship resources Positive screens addressed by a provider 120 • Overall, providers were satisfied with the tool & protocol Postive screens referred to survivorship resources 88 • Future projects include the development of a survivorship resources toolkit or further work to streamline the referral process • Implement a brief screening process to identify patients with unmet physical or psychosocial needs to increase the quality and quantity of screening Patients eligible for screening • Increase referrals to and, therefore, utilization of survivorship resources @uofunursing @utnurseresearch |
