Standardized, evidence-based management of sleep distrubances in high-risk incarcerated youths can be accomplished via adaptation & implementation of existing sleep management guidelines. .Background Results • Incarcerated youths have poor sleep & Utah Justice Youth Services (JJYS) providers lack sleep guidelines, resulting in inconsistent sleep management • Providers need guidelines including non-pharmacological & pharmacological interventions Methods Quality improvement project: 3 phases • Phase one: pre-implementation team interview revealed providers perceptions, preferences, & potential bias in sleep management • Phase two: guidelines were adapted & implementation began with an educational presentation for providers • Phase three: post-implementation team interview assessed the education & guidelines feasibility, usability, effectiveness, & satisfaction; a retrospective chart review measured a change in practice • Feasibility: guidelines & education are easy to follow & allow for quick decision making • Useability: guidelines were specific to JJYS setting & population; incorporation of provider feedback from pre-implementation promoted usability • Effectiveness: guidelines effective for basic sleep disturbances & standardizing sleep management in JJYS (yet insufficient evidence to conclude a practice change; 29.41% increase in guidelines use, p= .172) • Satisfaction: providers will continue to use the guidelines, along with revised sleep diary • Barriers: PRN staff unaware of current sleep management & lack of access to therapies (cognitive behavior therapy) Conclusion • Guidelines & education provide standardized, evidence-based sleep management for JJYS providers • Project success and future sustainability rely on sleep guidelines adherence by providers • Future projects include implementing universal sleep disturbance screenings for youths entering JJYS facilities • The sleep management project materials were disseminated to JJYS facilities @uofunursing @utnurseresearch |
