Show Effective Medication Education on Readmission Rates Among Heart Failure Patients Saja Alsarrieh, BSN Student Background Ø In the United States, readmission rates and hospitalization rates have been increasing in heart failure (HF) adult patients. 1 Ø Medication nonadherence is associated with an increase in readmission rates, morbidity, and mortality. 2 Ø The teach back method and having one-onone education with patients is a great way to improve discharge education success and to help lower readmission rates in HF patients. 3 Ø Having a discharge plan focused on medication adherence are effective in lowering readmission rates among HF patients. Outcome and Measurement Proposed Intervention Ø Emphasize thorough medication education along with verified patient understanding via the teach back method. Ø Increased adherence to medication use. Ø Lower percentage of readmission rates. Ø Measurement Tool: Readmission rates among HF patients at the UUH cardiovascular unit within 30 days after giving discharge instructions focused on medication use. Potential Barriers Ø Stakeholders remembering to use teach back method with discharge teaching. Ø Patients remembering to use medications as directed.
