Heart failure, a chronic and progressive disease, is notorious for high hospital readmission rates. The American Heart Association and the European Society of Cardiology advocate for self-care management education to mitigate these readmissions. Among the various educational strategies, the theory of empowerment emerges with a promising potential to enhance the efficacy of education, thereby improving patient outcomes. This quality improvement project, therefore, sought to identify the impact of self-care management education, employing methods rooted in empowerment theory on the 30-day readmission rate and overall outcomes for Veterans struggling with heart failure. |
