History A 50-year old male with 5-month history of flu-like illness characterized by mild fevers, frequent chills, night sweats, sinus congestion and diffuse myalgias in chest, abdomen, legs and soles of the feet. Three days PTA awoke unable to open his right eye. Diplopia with eye open. Previous history significant for Type II diabetes.
Pathology Acute infarcts in left anteromedial inferior thalamus extending into the uncus and left globus pallidus.
Disease/Diagnosis Meningovascular syphilis
Clinical Pupils: Dilated, fixed 7 mm OD, 2 mm reactive pupil OS; Eye movements: Complete ptosis and III nerve palsy OD.
Treatment Antibiotics
