Adenoid Cystic Carcinoma of Lacrimal Gland (Kennerdell Case 24, PPT)

History: A 20-year-old man presented with progressive painful right proptosis without functional deficit. Anatomy: Lacrimal Gland; Lacrimal Apparatus; Orbit. Disease Diagnosis: Carcinoma, Adenoid Cystic. Pathology: Fine needle aspiration of right orbital mass showing malignant Adenoid Cystic carcinoma of the Lacrimal Gland. Microscopic appearance of the Adenoid Cystic carcinoma invading and destroying bone. Presenting Symptom: Progressive painful right proptosis without functional deficit. Neuroimaging: B-Scan appearance of a regular mass in the upper temporal aspect of the right orbit behind the globe. Ultrasound-directed left to right shows the darkened lesion with variable internal reflectivity. CT shows right orbit a mass lesion eroding the lateral orbital wall and the anterior cranial wall extending in from the orbit into the sub-temporalis and intracranial space. Treatment: Cranial orbitotomy, surgical removal of carcinoma.
