History A 68-year old female with a 5-hour painless loss of vision OS followed by unremitting loss of vision 9 days after initial episode. Previous history significant for hypertension and diabetes mellitus.
Pathology Large, neoplastic lymphoid cells with prominent nucleoli and frequent mitotic figures consistent with intravascular lymphoma.
Disease/Diagnosis Intravascular lymphoma presenting as an anterior ischemic optic neuropathy
Clinical VA: 20/15 OD, 20/300 OS; Dyschromatopsia; RAPD OS; Near total loss of sensitivity to Humphrey visual fields OS; Mild temporal pallor OS
Presenting Symptom Painless, transient loss of vision OS
Neuroimaging MRI
Treatment R-CHOP therapy; Alkylating antineoplastic agents; Phytogenic antineoplastic agents; Immunosupressive agents; Glucocorticoids; Antineoplastic antibiotics
