Description Wernicke's Encephalopathy (WE) is a life threatening, reversible disorder classically characterized by a triad of encephalopathy, ataxia, and ophthalmic abnormalities. The most common ophthalmic manifestations of WE are (in order of frequency): gaze-evoked nystagmus, abducens palsy, and conjugate gaze palsies. A wide range of atypical afferent and efferent Neuro-ophthalmic findings have been reported in WE including: optic disc edema, peripapillary hemorrhages, ophthalmoplegia, primary position upbeat nystagmus, altered pupil reactivity and size, light-near dissociation, convergence spasm, and ptosis. WE has uncommonly been reported in pediatric patients. Here, we present a pediatric case of WE with bilateral optic disc edema and abducens palsy.
