Lack of Hemoglobin A1C (A1C) control in diabetes leads to increased neuropathy, nephropathy, ophthalmologic and cardiovascular disease.HEDIS measures define control for patients with Type 2 Diabetes as having an A1C less than 8%. Many factors contribute to A1C control, including ease of testing, education, weight loss and exercise, as well as collaboration with multiple providers including behavioral health, dietitians, pharmacists, care managers, primary care providers, and support staff. |
