Description Exclusion of aneurysm in isolated third nerve palsy has relied traditionally on intra-arterial digital subtraction angiography (DSA). Whether CTA or MRA can safely replace DSA depends on three factors: CTA/MRA sensitivity for aneurysms ? 3.0 mm, aneurysm prevalence in third nerve palsies and risk of DSA. This study quantifies these factors and provides current evidence-based guidelines for the use of DSA in third nerve palsies.
