Description Type 2 Diabetes (DM) is a leading cause of morbidity and mortality. Control of DM as measured by Hemoglobin A1c can reduce complications of the disease. This Quality-Improvement project aimed to increase the number of patients with A1c < 9.0 in a University based Family Medicine Practice.
