Description Pupil involving third nerve palsy traditionally raises concern for intracranial aneurysms. We describe an unusual case of a pupil involving third nerve palsy caused by a sphenoid sinus mucocele which have an incidence of only 1%. They usually cause headache followed by visual disturbance, and cranial neuropathies may occur in 50%.
