Description Medication therapy management (MTM) is an extended pharmacy service that optimizes therapeutic outcomes and goals for patients by ensuring that their medications are appropriate, effective, safe, and able to be taken as intended. By collaborating with Service Learning Scholars and Partner for Promotion, I aimed to implement an MTM service for Oquirrh Mountain Pharmacy. Oquirrh Mountain Pharmacy was selected because of its location within the Magna community. Magna is a diverse community with a low socioeconomic status. Due to its unique composition, the Magna community would benefit from an MTM service that could provide individualized care. As a result of implementing the service, community relationships were strengthened, many patients had their medication regimens optimized to their personal needs, and I learned many skills that will impact my future career as a clinical pharmacist.
