Description "UNSAFE OR HAZARDOUS CONDITION STANDARDS * REFERENCE COMMENTS Portable fire extinguishers had not been subjected to an annual maintenance inspection with a written record made, within the past one year. 1910.157(e)(3) The annual inspection is conducted by someone competent to service and repair the fire extinguishers. The right hand shaft on the Black and Decker bench grinder was exposed and not covered by a wheel guard or by a stationary shaft guard. 1910.219(0(4)0"") An unguarded revolving shaft can catch clothing or hair and cause an injury. A tool rest was not provided on the left hand wheel on the Black and Decker bench grinder. 1910.215(a)(4) A tongue guard was not provided on the left hand wheel on the Black and Decker bench grinder, and the wheel guard was located more than 1/4 inch from the wheel. .1910.215(b)(9) The electrical cord on the Black and Decker bench grinder was deteriorated. The deteriorated cord could create an electrical or fire hazard. The circuit breaker in the panel box located on the east wall, was not identified as to what circuits it controlled. 1910.303(f) Each breaker needs to be labeled showing which circuits it controls. A flexible electrical cord was used in place of an approved cable to provide conductors for one circuit coming from the circuit breaker panel located on the east wall. 1910.305(g)(1)(iii)(a) Flexible electrical cords are prohibited from being used in place of wiring that is approved for permanent wiring in a building. A splice was made in the circuit supplied by the flexible electrical cord. The splice was located high on the east wall, and the splice was not enclosed within a junction box. NEC 300-15(a) For most types of wiring, except for open wiring, splices are required to be placed in a junction box. Solvent contaminated rags were located in the northeast corner in the area of the paint storage. The rags were not stored in an enclosed metal container. 1910.106(e)(9)(iii) Solvent soaked rags present a fire hazard and they need to be kept in a closed metal container to minimize the fire potential. Flammable and combustible liquids were stored in an area within 25 feet of welding and cutting operations. 1910.106(e)(6)(i) Flammable and combustible liquids in an area where there is welding and cutting presents a fire hazard. The flammable liquids should be removed or stored in an approved cabinet. The emergency exit door located on the west wall was blocked by stored material. 1910.36(d)(1) In the sunnier, the overhead door provides an adequate exit. When the overhead door is closed, the personnel door needs to be clearly marked and ready for use. The emergency exit door located on the west wall was not marked as an exit. 1910.36(q)(1)"
