Description "UNSAFE OR HAZARDOUS CONDITION STANDARDS * REFERENCE COMMENTS Employees were not provided with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard is introduced into their work area, the employees have not been previously trained. 1910.1200(h)(1) A written standard operating procedure governing the selection and use of respirators had not been established for the workplace. 1910.134(0(1) Since emergency use respirators are required when handling the Magnacide, and respirators may be needed when painting in confined spaces, a written respirator program should be developed. A program administrator had not been assigned to administer the respiratory protection program. 1910.134(0(3} A North chemical cartridge respirator was stored in the welding rod storage cabinet on the west wall. The respirator was not stored to protect it from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals, and was not packed or stored to prevent deformation of the face piece and exhalation valve. 1910.134(h)(2)(i) To be effective and to keep from introducing new hazards, respirators need to be stored in a clean container. They also need to be protected from being"" physically damaged in storage. The two far west electrical receptacles on the south wall were improperly wired in that they had reversed polarity. 1910.304(a)(2) This is a code requirement, and in some cases, it could affect the safety of employees. The tool rest on the right hand wheel on the Jet bench grinder was not adjusted closely to the wheel with a maximum clearance of 1/8 inch. 1910.215(a)(4) The tool rest needs to be kept close to the wheel so that a tool being g dressed on the wheel will not bind between the wheel and the tool rest, which could cause the wheel to break or the operator's hand to contact the wheel. The tongue guard on the right hand wheel on the Jet bench grinder was not adjusted closely to the wheel with a maximum clearance of 1/4 inch 1910.215(b)(9) The tongue guard is designed to keep the opening between the guard and the wheel as tight as possible to prevent portions of the wheel from flying out if the wheel breaks during use. Portable fire extinguishers are not being visually inspected monthly to ensure that they are properly located, accessible, and will operate in case of need. 1910.157(e)(2) Need monthly inspections to ensure that fire extinguishers are accessible, have not been discharged, and generally are ready for use. The easiest way to document the inspection is to initial and date the fire extinguisher inspection tag."
