Description In 1949, Hench et al. (1) described the beneficial effects of systemic corticosteroids in the treatment of rheumatoid arthritis. Following this report, physicians began to consider other disorders, including acute optic neuritis, that might respond to similar treatment or to treatment with adrenocorticotrophic hormone (ACTH). The first systematic attempt to address this issue was a double-blind, prospective study; of the treatment of 50 patients with acute retrobulbar neuritis with either ACTH or placebo (2,3). The study showed that the group treated with ACTH recovered vision faster than the group give placebo, but at 1 year, the mean visual acuities in the two groups were not significantly different from each other.
