Abduction Deficit

This UPFRONT Case is a presentation of a 49-year-old female patient who was referred to the neuroophthalmology clinic with the complaint of diplopia at the left gaze.

Hwang MD, Rutgers New Abduction Deficit T his UPFRONT Case is a presentation of a 49-year-old female patient who was referred to the neuroophthalmology clinic with the complaint of diplopia at the left gaze. Although the pattern of ocular movements was suggesting a left sixth nerve palsy, the detailed examinations revealed a positive forced duction test and a subtle enophthalmos in the restricted eye. The orbital imaging demonstrated an infiltrative lesion. The subsequent studies included mammogram that revealed a breast tumor and a biopsy that confirmed a breast ductal adenocarcinoma. The metastatic nature of the orbital lesion was supported by a PET scan.
