Show UPFRONT Case Eric S. Hwang, MD Larry Frohman, MD Department of Ophthalmology and Visual Science, Rutgers New Jersey Medical School, Newark, New Jersey Address correspondence to Eric S. Hwang MD, Rutgers New Abduction Deficit T his UPFRONT Case is a presentation of a 49-year-old female patient who was referred to the neuroophthalmology clinic with the complaint of diplopia at the left gaze. Although the pattern of ocular movements was suggesting a left sixth nerve palsy, the detailed examinations revealed a positive forced duction test and a subtle enophthalmos in the restricted eye. The orbital imaging demonstrated an infiltrative lesion. The subsequent studies included mammogram that revealed a breast tumor and a biopsy that confirmed a breast ductal adenocarcinoma. The metastatic nature of the orbital lesion was supported by a PET scan.
