Description A healthy 19 year old man with normal body mass index presented with a one-year history of transient visual obscurations, severe headaches with pulsatile tinnitus, and intermittent episodes of numbness encompassing his lower face and distal extremities. Neurological examination was normal. Ophthalmologic examination demonstrated normal visual acuity and visual field, but bilateral optic disc edema (Figure1A-C). MRI brain/orbit and MRV demonstrated a stable left anterior temporal arachnoid cyst (Figure2). Lumbar puncture demonstrated an opening pressure of 42 cm H2O, 2 white blood cells (40% lymphocytes), protein 25 mg/dL, and glucose15 mg/dL.
