Description 17 year-old girl with anxiety, esophagitis (presumed GERD), subclinical hyperthyroidism and 3-month history of worsening abdominal pain presented with two weeks of acute nausea, vomiting and bloody stools. She was found to have acute pancreatitis and pancolitis based on elevated amylase and lipase and abdominal CT. Two days into her admission for work-up of inflammatory bowel disease (IBD), she reported acute onset, bilateral, vision loss in the setting of vomiting. Patient stated she lost her vision "looking out a car window" that day and had short term memory loss. She also endorsed tingling of her lips and fingertips a few hours after the onset of vision loss.
