The structure is concrete continuous frame bridge, located along SR-21 approximately seven miles west of the town of Beaver. The bridge has a maximum length of 75 feet and a maximum span length of 36 feet. It stands approximately 15 feet in height and has a span width of approximately 34 feet. Alterations to the bridge include the addition and changing ofthe railing and parapets (unknown date). The railing is currently fanned by a series of concrete K-rails on either side of the span. The bridge is in good condition it appears to receive regular maintenance 5 HISTORYIPROPOSED ACTION Describe the impending action (e.g., road widening, rehabilitation, alteration, demolition). A dditional historical information is optional. Bridge D-659 was built in 1960. It was conunissioned by the Utah State Road Commission and it was engineered and constructed by Thorn Construction Company, Inc. The bridge is a concrete continuous frame bridge which was a common style and design of bridge in the 1960s. The Utah State Road Conunission (now known as UDOT) still owns the bridge today.
