Description The midbrain and mesodiencephalic junction contain the key structures involved in the premotor control of vertical and torsional eye movements. Thus, vertical gaze palsy is characteristic of lesions involving the midbrain or mesodiencephalic junction. However, these area also have the neural structures for vergence and descending fibers controlling horizontal gaze. We report a patient with conjugate horizontal gaze palsy due to unilateral midbrain infarction.
