Description Periodic alternating gaze deviation (PAGD) is characterized by a horizontal eccentric gaze that lasts for a few minutes in one direction before reversing to the opposite direction with the cycle repeating itself. In children, it has been found in patients with cerebellar hypoplasia, and this supports the idea that the phenomenon typically is caused by cerebellar midline disease. However, PAGD is very rare in adults, and there have been few case reports.
