Description We report two cases of suspected progressive perineural spread (PNS) of facial squamous cell carcinoma affecting multiple unilateral cranial nerves. Upon radiographic investigation, there was a lack of evidence for PNS. We propose that treatment decisions can be made based on the clinical presentation with or without significant radiographic evidence of PNS of a known history of cancer.
