Description Transvenous embolization (TVE) is the primary option for endovascular treatment of carotid cavernous fistula (CCF) due to its safety and efficacy of obliteration of the fistula, but it can occasionally give rise to several complications including venous congestion, sinus perforation, and ocular motor nerve palsy. The ocular motor nerve palsy mostly occurred immediately after TVE procedure, while delayed onset of ocular motor nerve palsy has been rarely reported, and most of the reported cases had abducens nerve palsy. We describe a unique patient presenting delayed isolated oculomotor nerve palsy at eight years after successful coil embolization of CCF.
