Description Non-arteritic ischemic optic neuropathy (NAION) is the most common cause of ischemic optic neuropathy, however the exact pathogenesis is incompletely understood. Current understanding stems from models suggesting that microvascular insufficiency leads to hypoxia of the optic nerve(1). Anemia, usually in the setting of acute blood loss, has been rarely reported in association with NAION. We present a case of chronic, slowly progressive, anemia and underlying hepatorenal syndrome, leading to bilateral NAION.
