Description PPE room mixing was brought to our attention during the value culture huddle. This was result of clinical staff forgetting to update the room precaution type on Epic and often removing the precaution signs from the patient room's door after the discharge of the patient. This issue created, for a long time, a very stressful and unsafe working environment. Our staff had nonstop arguments between them and clinical staff and this concern had always caused delays of our patients' room turnover. After the identification of the problem, the visual checklist was created and all staff went through its training. Up to now, we are not hearing any complaints from the staff and arguments with clinical staff decreased. We are looking forward to working with our clinical staff partners to make more continuous improvement.
