History A 34 year-old woman was referred from the allergist for painless swelling of bilateral upper eyelids for 11 months, after; moving from Alabama to her current state of residence. Her past medical history was significant for anemia. Review of; systems was notable for fatigue, daily headaches, and intentional weight loss. Laboratory studies ordered by the referring; physician revealed a positive ANA (no titer), elevated ESR, elevated platelets, low hemoglobin; TSH, ACE, vitamin B12 and; folate, C4, and dsDNA were all normal. Her initial ophthalmology examination was remarkable for bilateral upper eyelid; edema with palpable lacrimal glands, mild bilateral upper lid ptosis, bilateral lagophthalmos, and exophthalmos left eye; greater than right. Visual acuity was OD 20/25, OS 20/20. Pupils were reactive with no relative afferent pupillary defect.; Color plates were normal. Extraocular movements were full and there was no lid lag in downgaze. The remainder of the slit; lamp and fundus examination were unremarkable.
