Description Peduncular Hallucinosis (PH) refers to dream-like visual hallucinations during wakefulness that are typically caused by a focal lesion in the rostral brainstem. The conventional understanding of PH is that associated eye movement abnormalities are frequently, if not always, present. However, in the era of modern Neuroimaging; PH has been increasingly diagnosed in patients without any objective deficits on physical examination. We sought to determine how modern neuroimaging has impacted the diagnosis of PH.
