Description A 26 year old man reported a three month history of episodic vertical diplopia. Spells typically occurred after prolonged downgaze and were most noticeable on upgaze to the right. They lasted up to a minute, occurred over 20 times daily, and were accompanied by a ""heavy"" feeling in the left eye.
