Description Two cases of periodic alternating gaze (PAG) and one case of 'ping-pong' gaze (PPG) will be described. In the two PAG cases, gaze was conjugately driven to the right, then to midline, and then to the left in cycles of 2 1/4 and 3 minutes. Alternating esotropia and overacting inferior oblique activity was present in one case manifesting periodic alternating esotropia. Nystagmus appeared only when attempting to fixate against the base-line tonic deviation. The PPG case demonstrated the same gaze pattern with a much shorter cycle length of 1-2 seconds.
