Description A 63 year old nurse noticed decreased vision in both eyes. She had an eight year history of diabetes mellitus, insulin-dependent for three years. She was mildly hypertensive and six months earlier she had a hemispheral TIA with double vision and slurring of speech but with no sequelae. Visual acuity was 20/40 RE and 20/30 LE. At first, maculopathy was suspected but fluorescein angiography was normal. Acuity decreased further during the ensuing three months to 20/200 and 20/40, respectively.
