Description A 31 year old woman with AIDS, transferred from another hospital presented with an examination notable for lethargy, bilateral esotropia, absent upgaze, convergence retraction on attempted upgaze, pupillary light-near dissociation and lid retraction, consistent with a pretectal syndrome. Evaluation included contrasted cranial CT consistent with diffuse atrophy, and gadolinium enhanced cranial MRI without evidence of hydrocephalus.
