Description The role for early neuroimaging in patients older than 50 years presenting with isolated 3rd, 4th or 6th nerve palsy remains controversial. While some authors recommend waiting 3-6 months for spontaneous resolution to occur before imaging older patients with isolated 3rd, 4th, or 6th nerve palsies, others recommend that MRI and other studies be performed early.
