Description Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is an inherited generalized nonatherosclerotic nonamyloid arteriopathy, characterized by migraine, early onset stroke, and finally subcortical vascular dementia. The causative mutations are identified in the Notch3 gene (NOTCH3). We describe the clinical course and fluorescein angiographic features of a case of CADASIL syndrome and demonstrate an association between retinal vascular changes and cerebral small vessel alterations in CADASIL.
