Description Bilateral lid retraction, or Colliers sign, is a common finding in patients with dorsal midbrain injury, presumably due to disruption of inhibitory fibers to the central caudate nucleus (CCN). Patients with medial midbrain lesions sometimes demonstrate a plus-minus lid syndrome (PMLS) with ipsilateral ptosis and contralateral lid retraction.
