Description History: Patient presented with painful left proptosis with a blind downward displaced eye for two weeks. He had a known history of gastric carcinoma with a gastric partial resection. Anatomy: Orbit. Disease Diagnosis: Adenocarcinoma, Mucinous; Metastatic. Etiology: Gastric carcinoma. Neuroimaging: Ultrasonic appearance of a mass behind the left eye. The ultrasound directed from left to right with a posterior wall of the eye seen and a round anechoic (solid mass) with a rounded posterior wall behind the left globe. Axial CT scan showing a lesion behind the left eye with resultant proptosis. A higher axial cut showing the lesion in the superior anterior left orbit. Pathology: A Papanicolaou stain needle biopsy showing the malignant mucinous adenocarcinoma cells. A cell block made from the needle biopsy further demonstrating the malignant mucinous adenocarcinoma metastasized from the gastrointestinal tract to the left orbit. Presenting Symptom: Painful left proptosis with a blind downward displaced eye for two weeks. Treatment: Left orbital exoneration.
