History An 80-year old male with left periorbital and temporal pain followed within one day by left ptosis and diplopia. Previous history significant for coronary artery disease, hypertension and abdominal aortic aneurysm.
Pathology Pleomorphic cells, Giant cells with foamy cytoplasm with areas of necrosis and neutrophils in the cavernous sinus consistent with histiocytoma.
Disease/Diagnosis Primary interacranial malignant fibrous histiocytoma arising in the cavernous sinus
Clinical VA: 20/20 OD, 20/40 OS; Color plates were bilaterally symmetrical
Presenting Symptom Left periorbital and temporal pain; Left ptosis; Diplopia
Neuroimaging CT; MRI
