Description Chronic maxillary atelectasis (CMA), also referred to as silent sinus syndrome when occurring in the absence of known sinus disease, is characterized by an insidious, progressive collapse of the maxillary sinus and downward bowing of the orbital floor presenting with facial asymmetry, enophthalmos, hypoglobus, and occasional diplopia1. We report a case of CMA presenting with progressive ipsilateral hypotropia and mimicking a contralateral cranial nerve 4 palsy.
